Compliance Summary Remarks:
Well is present with a submersible pump. Flow line is connected. Well is inactive due to a leak in flow line approximately 150 yards from well location. Flow line is above ground, parallel to Lease road up to Co. Rd. 87. Flow line runs parallel to Co. Rd. 87 for approximately 150 yards. Flow line is then buried under Co. Rd. 87. Its above ground at the intersection of Co. Rd 87 and a Lease road leading to other well locations, for 10 feet. The flow line then runs above ground parallel to Co. Rd. 87 , for approximately 150 yards to tank battery location. The leak was approximately 10 feet in diameter and 10 feet high on vegetation. Stained vegetation was removed as well as surface soil and replaced with new soil. Flow line was fused back together and well was activated with no leaks. Clean up was completed on this date.
