Compliance Summary Remarks:
Fluid level test conducted on 08/13 = 3530' @ 120 PSI, pump jack at wellhead, no tubing test conducted. NOTE: Location has been completely under water, due to heavy floods. Lease road completely destroyed, walk in or ATV access, 15 bbl used oil tank and two 300 gal oil tanks were overturned, 15 bbl plastic tank and one of the 300 gal metal tanks is in a wooded area off of the location, produced water tank was moved inside the containment and containment now has a large amount of sand inside it, all fencing around the equipment was heavily damaged. No enforcement at this time.MP
