Compliance Summary Remarks:
Prism has this well listed as a production well unable to access the well location but was able to view well location and take pictures from Mt. Zion Road the fluid tank is almost full of storm water and the secondary containment also has storm water in it the flow meter for this well which has been located on an adjacent well location (Lucille #1-16) is no longer there.
