If yes to any, explain:	This is an orphaned well that has been plugged. The well was re-visited because I had to go to the location to fix another well. The wellhead stands about 2' out of the ground and is filled with cement. It will have to be cut below ground level capped and buried. The associated surface equipment (separator, etc.) remains on-site and will need to be removed when the nearby orphaned wells are plugged.
