Compliance Summary Remarks:
Prism has this well listed as a producing well. Valve are shut in on well head and not open to flow gas but there is an odor of gas coming from wellhead. Excess vegetation around equipment Containment tank is halfway full and the secondary containment is almost full of storm water. No meter on site. Now AOW well.
